Portuguese TDI Student Record Folder
The Technical Diving International Student Record Folder gives each student an area to record personal information, emergency contacts, dive training record, medical information, and finally general liability as well as an express assumption of risk. The student record folder is valid for thirty days after it is originally signed and dated by the student's instructor.
